American Hospice and Home Health Care Services (AHHHC) implements programs and services to provide comprehensive quality health care in the patient’s home. We address each patient’s unique psychological, safety, self-esteem, and self-actualization needs in a client based plan of home health care.

Our company serves as a resource, providing health care education for the client, the client's family, and the community . These services are provided by our very skilled and well trained team of Registered Nurses, Licensed Vocational Nurses, Physical, Occupational and Speech Therapists, Medical Social Workers, and Home Health Aides.

AHHHC provides developmental opportunities and programs, including orientation and programs concerning current practices in nursing and allied health care for all staff and maintains open communication with clients, employees, and physicians via frequent contact in order to achieve quality and cost-effective care. We provide current and accurate personnel records, including verification of current license/certification and health records.

AHHHC provides clinical services (either through its own staff or through contract) from Registered Nurses (“RN”), Licensed Vocational Nurses (“LVN”s), Home Health Aides (“HHA”), Physical Therapists (“PT”), Occupational Therapists (“OT”), Speech Therapists (“SP”), Medical Social Workers (“MSW”) and Pharmacists.

AHHHC services Alameda, Contra Costa, Stanislaus, San Joaquin, Solano, Santa Clara, and San Mateo counties. We continue to adhere to standards set forth by Federal, State, Local and Accrediting Bodies.
