POSITION: COMMUNITY HEALTH REGISTERED NURSE

DUTIES:

To provide nursing care in accordance with the client's plan of care to include comprehensive health and psychosocial evaluation, monitoring of the client's condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care.

POSITION: ADMINISTRATOR & DON

DUTIES:

1. Administers directs and coordinates all administrative and financial activities of the Agency
2. Assists with the planning, coordination, controlling and evaluation of home health services
3. Responsible for the development and implementation of policies and procedures, for obtaining input from service personnel and providing professional input to related administrative systems
4. Directs and supervises professional and auxiliary personnel rendering client care services
5. Performs marketing duties and fosters public relations

POSITION: ALTERANTE DON

DUTIES:

In the absence of the Director of Nursing, the Alternate Director of Nursing, under the supervision of the Administrator, assists with the planning, coordination, leading, controlling and evaluation of home health services.

POSITION: ALTERNATE ADMIN & DON

DUTIES:

In the absence of the Administrator/Director of Nursing, directs and coordinates all administrative and financial activities of the Agency.

POSITION: BUSINESS CLERK

DUTIES:

Performs routine office clerical tasks.

POSITION: CHIEF EXECUTIVE OFFICER

DUTIES:

1. Implementing governing body directives or policies
2. Overseeing fiscal management
3. Complying with applicable laws and regulations
4. Monitoring quality and appropriateness of services and products and assuring their availability
5. Program planning, development, administration and evaluation
6. Overall administration of Agency and its affiliated companies
7. Coordinating and liaising with appropriate affiliate departments and committees
8. Representing the organization to other groups, agencies, and the general public
9. Keeping the governing body and staff informed of current organizational, community, and industry trends
10. Developing policies and guidelines of operations
11. Seeking and encouraging input from other administrative and service personnel
12. Providing professional input to related administrative systems

POSITION: CHIEF FINANCIAL OFFICE

DUTIES:

Organizes and administers the business and financial activities of the Agency, advises the CEO and Administrator about financial trends affecting the Agency.

POSITION: COORDINATOR

DUTIES:

Directs the coordination of patient care under the supervision of the Director of Nursing.

POSITION: COURIER & CLERK

DUTIES:

Acts as a courier for the Agency delivery and picking up packages as needed provides other clerical support.

POSITION: DIRECTOR OF HUMAN RESOURCES

DUTIES:

Is responsible for managing the Human Resources Department and overseeing all personnel issues.

POSITION: DIRECTOR OF NURSING

DUTIES:

1. The Director of Nursing (DON), in accordance with policies and standards established by the Board of Directors, and under the direct supervision of the Administrator, assists with the planning, coordination, leading, controlling and evaluation of home health services
2. The DON is responsible for coordination and liaison with appropriate parent and affiliate Departments and committees, development of policies and guidelines for and obtaining input from service personnel and providing professional input to related administrative systems
3. The DON directs and supervises professional and auxiliary personnel rendering client care services, performs marketing duties and fosters public relations for the company, consults and plans with health care facilities regarding staffing needs, and communicates the concept of quality client care to the general community
4. The DON interprets service needs to the Administrator and acts as a client services administrative liaison.

POSITION: HOME HEALTH AIDE

DUTIES:

1. Provides personal care and related services in the home, under the direction, instruction and supervision of the staff nurse and the Director of Nursing
2. Tasks to be performed by an HHA must be assigned by and performed under the supervision of an RN who will be responsible for the patient care provided by the HHA
3. Under no circumstances may an HHA be assigned to receive or reduce any intravenous procedures, procedures involving the use of Levin's tubes or Foley catheters, or any other sterile or invasive procedures.

POSITION: LICENSED VOCATIONAL NURSE

DUTIES:

1. Provides skilled nursing care under the direct supervision of the Registered Nurse
2. Performs selected skilled actions in the provision of curative, rehabilitative, palliative or preventative nursing care
3. Is responsible and accountable for making decisions that are based on the individual's nursing experience and educational preparation

POSITION: MEDICAL DIRECTOR

DUTIES:

Is a part-time employee appointed to provide direction, professional support, guidance, medical and technical assistance to the clients and staff of the Home Health Agency.

POSITION: MEDICAL RECORDS MANAGER

DUTIES:

Is responsible for maintaining records for past and present clients.

POSITION: MEDICAL SOCIAL WORKER

DUTIES:

Provides medical social services to clients in their homes in accordance with physician orders and under the direction and supervision of the Director of Nursing or another appropriate supervisor.

POSITION: NURSING SUPERVISOR

DUTIES:

Maintains the quality of client care in accordance with the Agency's objectives and policies, through planning, coordination, and implementation and evaluation of the home health services provided.

POSITION: OFFICE COORDINATOR

DUTIES:

Oversees the operations of the office, including maintaining current employee personnel files, answering all phone calls, preparing and maintaining the Agency's payroll system, and compiling statistics necessary for the Administrator.

POSITION: OCCUPATIONAL THERAPIST

DUTIES:

Provides skilled occupational therapy service on an intermittent basis to clients in their homes in accordance with the physician's orders.

POSITION: OCCUPATIONAL THERAPIST ASSISTANT

DUTIES:

Provides skilled occupational therapy service on an intermittent basis to clients in their homes under the supervision of the Occupational Therapist.

POSITION: PAYROLL & BILLING & RECEIVABLES CLERK

DUTIES:

Assists the Payroll/Billing/Receivables Manager in all aspects of payroll and billing.

POSITION: PAYROLL & BILLING & RECEIVABLES MANAGER

DUTIES:

Manages the Agency's payroll, bill payments and receivable collections.

POSITION: PHYSICAL THERAPIST

DUTIES:

Provides skilled Physical Therapy services on an intermittent basis to clients in their homes in accordance with physician orders.

POSITION: PHYSICAL THERAPIST ASSISTANT

DUTIES:

Provides skilled Physical Therapy services on an intermittent basis to clients in their homes under direction form the Registered Physical Therapist.

POSITION: SPEECH THERAPIST

DUTIES:

Provides skilled speech pathology service on an intermittent basis to clients in their homes in accordance with the physician's orders.

POSITION: STAFFING COORDINATOR

DUTIES:

1. Is responsible for the coordination of patient visits, maintenance and upkeep of scheduling records and log books
2. Staffing coordination duties include the accurate and timely communication of scheduling changes between office and field staff.

POSITION: UNLICENSED INTAKE COORDINATOR

DUTIES:

Assists in the coordination of patient care under the direction of the Coordinator.
