A specially trained MD manages and coordinates patient care. Each case is reviewed and updated every two weeks or more often as necessary.

Hospice nurses provide individualized nursing care which may include wound care, medication management, IV therapy, catheter care and much more. Hospice nursing focuses on end of life and comfort. There is always a hospice nurse on-call 24/7.

Provide one on one assistance for hygiene, bathing, mouth care, dressing, light housekeeping and much more.

Psychological Services (Medical Social Worker)

Psychological Services (Medical Social Worker)

Are provided by trained Medical Social Workers to patients and their loved ones. This may include financial and additional community resources.

Financial and Community Resource Referrals

Financial and Community Resource Referrals

Working with patients and their loved ones, MSW’s provide psychological counseling, assist in referring community resources and address financial concerns.

Spiritual Care Counseling(Hospice Chaplain)

Spiritual Care Counseling(Hospice Chaplain)

A hospice chaplain coordinates care addressing the spiritual needs of patients and their families. If the patient and family choose their own clergy, the hospice chaplain is a resource to clergy.

Train volunteers provide comfort and support to patients and families.

Rehabilitative Services

Rehabilitative Services

Utilize physical therapy, occupational therapy and speech therapy to provide strengthening, pain management, and gentle exercise.

Speech Therapy Services (Speech Therapist)

Provide individualized therapy with focus on mouth and tongue. This includes safe swallowing practices, speech articulation, word recognition and much more. Each visit lasts about an hour and frequency is determined as needed.

Nutritional Services (Registered Dietitian)

Provide individualized dietary advice and guidance. In addressing weight loss or gain, meal planning, disease management and supplementation, Registered Dieticians are an integral part of the care team.

In times of crisis, the hospice team provides one on one around the clock care.

provided to those patients admitted to the hospital.

Respite Care

Respite Care

Provision of short-term accommodation in a facility outside the home in which a loved one may be placed (upto 5 days). This provides temporary relief to those who are caring for family members, who might otherwise require permanent placement in a facility outside the home.

Pharmacy Services

Pharmacy Services

All hospice medications are provided free of charge to the patients. Medication reconciliation is provided to optimize comfort.

Durable Medical Equipment & Supplies

Durable Medical Equipment & Supplies

DME may include oxygen, hospital bed, wheelchair or commode. Supplies may include gloves, diapers, wound care supplies, IV fluids and skin care products. All is provided free of charge as needed.

Transportation Services

Transportation Services

Assists and provides transportation to appointments as needed by patients and family members.
