Wound Care

Often referred to as “visiting nurses”, our team of highly trained nurses, RN and LVN’s, provide routine and complex care and education, including Post-Surgical care, Wound Care, I.V Therapy, Disease Management, Medication instruction/administration, catheter care, lab work and much more. The nurses usually spend 45-60 minutes at each visit and come as frequently as necessary. Together with the patient and their families, Nurses develop and coordinate the plan of care with their physician for optimum outcomes.

Hospice nurses provide individualized nursing care which may include wound care, medication management, IV therapy, catheter care and much more. Hospice nursing focuses on end of life and comfort. There is always a hospice nurse on-call 24/7.

Physical Therapy

Registered Nurse

Provide patient specific exercise and therapy for patients to regain their previous level of function. They will also develop a long term home exercise program to prevent relapse. Patients routinely benefit from PT following hospitalization, surgery, joint replacement, recent falls or general deconditioning/decline. Each visit lasts about an hour and frequency is determined as needed.

Focused approach to assist patients regain or improve their level of function. This may include dressing, bathing, feeding and much more. Each visit lasts about an hour and frequency is determined as needed.
